Privacy-sensitive data / personal data are processed by Statenkliniek.nl (a website of Stichting Statenkliniek (a foundation, hereunder called StatenClinic), located at Frankenslag 357, NL-2582 HP Den Haag). StatenClinic considers careful handling of personal data very important. Personal data is therefore carefully processed and secured by us. Statenkliniek.nl is hosted by Heeg Internet Solutions BV, PO Box 479, NL-1940 AL Beverwijk sub-contracted by ToTabIT, Dorpsstraat 47 NL-3209 AD Hekelingen (Spijkenisse).

In our processing we comply with the requirements set by the EU - General Data Protection Regulation (GDPR). That means, among other things, that:

• we clearly state why we process personal data. We do this via this privacy statement;
• we limit our collection of personal data to the personal data necessary for our statutory purposes;
• we ask you for explicit permission first to process your personal data in cases where your consent is required;
• we take appropriate security measures to protect your personal data and we also require appropriate measures from the parties that process personal data on our behalf;

If you use our website Statenkliniek.nl, you can send the following personal data via a contact web form. (Information marked with * is required for sending the web form):

• your title, first name and last name *;
• your date of birth*;
• your address*;
• your telephone number* and e-mail;
• your Dutch health insurer*;
• your Genarl Practitioner;
• your BSN (Dutch social security number);
• information with whom you want to have an appointment;
• your comments and
• your statement that you agree with us using your data*.

We process your (medical) personal data for the purpose of making an appointment and for creating a record in the digital patient file of StatenClinic. We share personal data only with parties with have been authorised by us. This privacy statement explains how this happens.

As legally required, StatenClinic will keep your medical records at least 15 years after your treatment has ended.

If you fill out our contact form on the website or if you send us an e-mail, we use your data to contact you (for making an appointment and to initiate an entry in our data base, see above) or to reply to your e-mail. We will not provide your e-mail address to any mailing list system. If you want to stay informed about our services, we offer you to subscribe to a RSS feed.

We do not use cookies on statenkliniek.nl. Our website does not use tracking software nor does it use analytics software.

We take appropriate security measures to limit misuse of, and unauthorized access to, your personal data. In this way we ensure that only the authorised persons (subject to medical confidentiality) have access to your data, that access to the data is protected and that our safety measures are checked regularly.

To protect your data during transmission, we use state-of-the art encryption, SSL via HTTPS.

Downloadable documents, files or media that are made available on this website are provided to users at their own risk. Although every precaution has been taken to ensure that only genuine downloads are available, users are advised to verify their authenticity using third-party antivirus software or similar applications.

This privacy statement does not apply to websites of third parties that are connected to this website by means of links. We can not guarantee that these third parties handle your personal data in a reliable or secure manner. We encourage you to read the privacy statement of these websites before using these websites. The Statenkliniek.nl website does not contain sponsored links and advertisements. We accept no responsibility for third party downloads and downloads by external websites and advise users to verify their authenticity using third-party antivirus software or similar applications.

We do not use social network buttons, but our own icons that link to the respective Facebook, Google+ and LinkedIn website and to the LinkedIn profiles of our employees. We link to Google Maps in different places on our website. Check first if you want to be logged in to the respective services before clicking on these links. For customer feedback we use a button and a widget from ZorgkaartNederland.nl, by the use of which you are forwarded from our website to ZorgkaartNederland.nl for the assessment of StatenClinic, where "Statenkliniek" is used as a search term on their website.

We reserve the right to make changes to this privacy statement. It is advisable to consult this privacy statement regularly so that you are aware of these changes.

Of course we will help you further if you have complaints about the processing of your personal data. You can reach us via one of the contact contacts mentioned in our imprint. After that, if you are still dissatisfied with the way we handle your complaint, you have the right to file a complaint with the Dutch Data Protection Authority.
